Better recovery happens when healthcare works together.
A patient leaves hospital, and rehabilitation either continues or it does not. We build the routes that make sure it does: clear referral pathways, honest reporting back, and one care plan the whole team can see.
Who we work with
Six kinds of partnership.
Each one is written down before it starts: what we do, what you do, what gets reported back and how often.

Hospitals
Referral pathways, post-discharge rehabilitation and specialist rehabilitation, so a patient discharged on Friday is not still waiting three weeks later.

Doctors
Referral collaboration, shared care and clinical communication. You refer, you receive the assessment and the discharge summary, and you can ask for an update at any point.

Insurance companies
Approved rehabilitation pathways, coordination of a member's care, and the reporting an insurer needs to authorise and review treatment.

Employers
Workplace rehabilitation, return-to-work programmes and ergonomics, aimed at getting a valued employee back to doing their job safely.

Schools
School-based rehabilitation, support for inclusive education, and work with children who have additional physical needs.

Care facilities
Geriatric rehabilitation, mobility programmes for residents, and practical training for the staff who provide day-to-day care.
We describe only what we can currently deliver from our centre in Kumasi and through our homecare service. If a partnership needs more than that, we will say so before it is agreed.
How a partnership runs
Agreed, then kept.
A partnership is only as good as what happens after the first meeting, so the working parts are simple and repeatable.

Agree
One conversation, then a written pathway: who is referred, how quickly they are seen, what is reported and to whom.

Refer
Referrals come in through your own space in Haven Connect, or by WhatsApp and email if that suits you better.
- Step 3
Report
Assessment findings, the rehabilitation plan and discharge summaries go back to you, with the patient's consent.
- Step 4
Review
We meet on a schedule you set, look at what happened to the people you sent us, and fix what is not working.

Each partner organisation can be given a private, signed-in space, so the working relationship does not live in scattered messages.

Send a referral, see its status, and follow what happened at assessment, during treatment and at discharge.

Shared documents, rehabilitation updates and reports, held privately and never published anywhere on this website.

Upcoming meetings, case discussion, and a direct line to the clinical team rather than a general enquiry address.
Patient information is shared only where consent and professional permissions allow it, and every partner space is separate from every other.

Partners are welcome to visit the centre at 15 Crescent Avenue, South Patasi, Kumasi, Monday to Saturday between 8:00am and 6:00pm. Please arrange it with us first so a clinician is free to show you round.
